For mail requests, download the Birth Abstract Request Form from the CCCHD website, fill it out, and send it with a cashier's check or money order for $24.00 made out to CCCHD. Include a self-addressed stamped envelope. Out-of-state checks are not accepted. Certified copies ship the next business day after the office gets your request.
